Comprehensive Medication Management Clinic

What is Comprehensive Medication Management (CMM)?

CMM is a service that focuses on you, the patient. Clinical pharmacists work with your other healthcare providers to make sure that your medications are right for you, work well, are safe, and are taken the way they should be to help meet your health goals, especially for long-term conditions. Parkview offers several locations where patients can get help managing their anticoagulants (blood thinners)—including warfarin and direct oral anticoagulants such as Eliquis and Xarelto—and various long-term conditions. Our team will make sure that blood levels and additional labs are checked at regular intervals and will adjust your medication doses when needed to achieve your goals.

How can I schedule an appointment at the CMM Clinic?

Before you can schedule an appointment at the CMM Clinic, your doctor must first send a referral. A team member will then contact you to schedule your initial appointment at a location that is most convenient for you. Please refer to our clinic locations to find your preferred location. If you need to change your appointment, please call your selected clinic as soon as possible. The CMM Clinics see patients by appointment only.

What can I expect at my CMM Clinic appointment?

During your appointment at Parkview’s CMM Clinic, you will meet with a qualified pharmacist. At your first appointment, the pharmacist will review your medical history and medication list (including all prescriptions, over-the-counter drugs, and supplements) and provide education regarding your medication. If you take warfarin, the pharmacist will check your international normalized ratio during each visit by performing a finger-stick blood test. If you take other blood thinners, such as Eliquis or Xarelto, or other chronic disease state therapies, you may need to go to the lab at least 24 hours prior to your appointment to complete updated lab work that will be reviewed by the pharmacist during your appointment. Your medication doses may be changed as needed based on your test results. Before you leave, you will be given a date for your next visit.

Warfarin therapy: Patient self-testing program

Parkview also offers a self-testing program through the CMM Clinic for individuals on warfarin who meet specified criteria that allow them to perform these tests at home. There are several benefits of testing at home, including reducing travel time and saving money spent on gas. You may qualify for the program if you have a qualifying diagnosis and meet the following criteria:

  1. 1. You have been taking warfarin for at least 90 days
  2. 2. You will be taking warfarin for at least one year
  3. 3. You—or a caregiver—are willing to perform weekly finger-stick tests
