The Parkview Advantage Clinic (PAC) offers a team-based approach to address the medical and social needs of patients with two or more chronic conditions who have Medicare Advantage plans: United Healthcare (UHC), Humana or Aetna. At the PAC, your care will be provided by a team of two physicians, a full-time pharmacist, a licensed clinical behavioral therapist and a nurse care navigator.
